چکیده

Introduction Epicardial mapping revealed the existence of abnormal potentials on the right ventricular outflow tract (RVOT) in patients with Brugada syndrome. It is reported that tissues of the RVOT epicardium from open-chest biopsy and autopsy showed the existence of fibrosis and fatty infiltration associated with increased interstitial collagen and decreased connexin 43. Although the abnormal potentials can be explained by repolarization abnormality of delayed upstroke of the action potential dome and concealed phase 2 reentry, these abnormal potentials are associated with epicardial tissue fibrosis and include the mechanism of delayed conduction. We experienced a case of Brugada syndrome and frequent ventricular fibrillation (VF) episodes. The patient had abnormal epicardial delayed potentials that have been reported previously and also an unusual isolated very delayed potential (IVDP) that was reproducibly recorded 400–500 ms after the last ventricular activation on the epicardium of the RVOT.

Local Left Ventricular Epicardial J Waves and Late Potentials in Brugada Syndrome Patients with Inferolateral Early Repolarization Pattern

Background: Brugada syndrome (BrS) is characterized by J-point or ST-segment elevation on electrocardiograms (ECGs) and increased risk of ventricular fibrillation (VF). In BrS, epicardial depolarization abnormality with delayed potential on the right ventricular outflow tract is reportedly the predominant mechanism underlying VF. Yet VF occurrence is also associated with early repolarization (E...

Basis for the Induction of Tissue-Level Phase-2 Reentry as a Repolarization Disorder in the Brugada Syndrome

AIMS Human action potentials in the Brugada syndrome have been characterized by delayed or even complete loss of dome formation, especially in the right ventricular epicardial layers. Such a repolarization pattern is believed to trigger phase-2 reentry (P2R); however, little is known about the conditions necessary for its initiation. This study aims to determine the specific mechanisms that fac...

T wave alternans in an in vitro canine tissue model of Brugada syndrome.

Macroscopic T wave alternans (TWA) associated with increased occurrence of ventricular arrhythmias has been reported in patients with Brugada syndrome. However, the mechanisms in this syndrome are still unclear. We evaluated the hypothesis that TWA in Brugada syndrome was caused by the dynamic instability and heterogeneity of action potentials (APs) in the right ventricle. Using an optical mapp...

Arrhythmia/Electrophysiology Prevention of Ventricular Fibrillation Episodes in Brugada Syndrome by Catheter Ablation Over the Anterior Right Ventricular Outflow Tract Epicardium

Background—The underlying electrophysiological mechanism that causes an abnormal ECG pattern and ventricular tachycardia/ventricular fibrillation (VT/VF) in patients with the Brugada syndrome (BrS) remains unelucidated. However, several studies have indicated that the right ventricular outflow tract (RVOT) is likely to be the site of electrophysiological substrate. We hypothesized that in patie...
